Job Title: Equity Champion
Job Summary: The Equity Champion is responsible for promoting fair and equitable access to healthcare by identifying and addressing any disparities or barriers that may exist among our patient population and bringing these to the attention of the team. A focus on ensuring that all individuals receive equitable care and have equal opportunities to access our services is our goal. The Equity Champion will collaborate with the healthcare team to develop and implement strategies that foster inclusivity, eliminate bias, and promote cultural competence.
Responsibilities:
Assess and Evaluate:
Conduct periodic assessments to identify potential disparities or inequities in healthcare access, outcomes, and patient experiences.
Analyze data and trends related to patient demographics, health outcomes, and healthcare disparities to inform decision-making and develop targeted interventions.
Evaluate existing policies, practices, and protocols to identify areas for improvement in equity and inclusivity.
Strategy Development and Implementation:
Collaborate with the healthcare team to develop and implement strategies that promote equitable access to healthcare services.
Advocate for policies and procedures that address systemic barriers and promote diversity, inclusion, and cultural competence within the practice.
Support the development and implementation of training programs to enhance staff awareness and sensitivity to issues of equity and bias.
Patient Engagement and Education:
Collaborate with community organizations and resources to provide educational materials and resources that address health disparities and social determinants of health.
Advocate for patients' rights to receive equitable care and facilitate communication between patients and healthcare providers.
Collaboration and Communication:
Serve as a liaison between the practice and community organizations to facilitate collaborations and partnerships that address healthcare disparities.
Communicate regularly with the healthcare team to provide updates on equity initiatives and share best practices.
